Brevard Health Alliance, Inc. is Brevard County’s only Federally Qualified Health Center. BHA provides extensive primary health care services to Brevard County residents regardless of their ability to pay.
By offering services on a sliding-fee scale, BHA has provided primary care service, behavioral health services, limited dental services, diagnostic services, resource management services, medication assistance, and specialty referrals to Brevard County’s economically disadvantaged residents since March of 2005.
Objective
Our objective is to provide a primary health care home for our patients. As your family physician, our goal is to provide not just urgent care, but preventive care and health care related education to our patients.
Brevard Health Alliance is a private, not-for-profit organization with a community board of directors. More than one-half of our board members are patients of our clinics. We currently operate a pediatric clinic and adult clinic in Palm Bay, Malabar, Rockledge, Melbourne and Titusville.
In addition to these locations, our flagship clinic is located on Sarno Road and includes all specialties (pediatric, adult, behavioral health, and dental).
Mobile Unit
Our Mobile Health Units provide services to citizens at various locations throughout Brevard County. The goal of Brevard Health Alliance Mobile Health Units is to provide professional medical care to Brevard’s residents ‘where they are’; on the streets, in shelters, at soup kitchens and at drop-in centers. We currently provide services to approximately 2,500 Brevard County citizens that present as homeless.
Excellent Medical Care
The Brevard Health Alliance provides primary medical care with board certified family practice and internal medicine physicians as well as advanced practice nurse practitioners and physician assistants. This care includes health maintenance such as, well child check-ups, well woman care, and physicals. BHA also provides chronic disease management for conditions including hypertension and diabetes. Brevard Health Alliance is an FTCA Deemed Facility. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
Since opening on March 7, 2005, The Brevard Health Alliance has:

- Provided more than 32,000 dental visits
- Provided more than 17,250 behavioral health visits

| We are a non-profit corporation registered under the laws of the State of Florida. Our board of directors consists of people selected from several different denominational backgrounds and independent churches. They and all the others involved with Prison Book Project donate their time and individual skills.
We have partnered with several Christian Publishers who send us returns,overstocks & damaged materials. Some publishers require that we pay the incoming freight cost;about $500 per pallet with a discount. We mix up this material to ensure a good assortment and send it out to build chapel libraries and book carts in prisons and jails.
Publishers send inmate mail that they receive to us which we use to build our database and whenever possible, we try to take care of individual requests. Mailing to chaplains is the most preferable way of getting books into facilities. We can only mail 12 books in packages to individuals for the cost of one box with 65 books to a chaplain. In addition, our tests indicate that a book placed in a sharing environment, such as a chapel library, will be read by at least 30 people while a book sent to individuals will usually be read by five or less.
We normally ship one box per 500 inmates. Rotating through the states alphabetically, we usually get to everyone once a year, depending on how much money comes in. We
would like to be able to send to everyone twice a year; but, due to a lack of shipping funds and insufficient space we have been unable to reach that goal yet.
Letters are important and encouraging to us. When we receive especially heartwarming letters, we try to send copies back to our book providers. Prison Book Project is a team consisting of publishers, chaplains, inmates and volunteers who labor unselfishly in our distribution center. Seeing the gratitude of prisoners who have been blessed motivates us all the more in our efforts. Thank you letters reflect the love of touched lives and we often receive testimonies of how these books have led people to Jesus. The Brevard Alzheimer's Foundation Story and Time line
Founded in 1995, The Brevard Alzheimer's Foundation is a not-for-profit 501(c)(3) organization. Donations and services stay in Brevard County, serve local residents and are fully tax-deductible. With an annual operating budget of $4.9 million dollars and over 80 employees, the Brevard Alzheimer's Foundation served over 2,000 families last year. Our organization is unique among other charitable organizations because over 90% of our revenue comes directly from services provided and not from fundraising.

| Brevard Family Partnership was established in 2003 as a non-profit organization in response to the Legislative mandate to privatize foster care and related services (Section 409.1671, Florida Statutes).
The Agency was subsequently awarded the contract, through the Florida Department of Children and Families to manage Brevard County’s child welfare system. Brevard Family Partnership and its community partners provide child abuse prevention, foster care and support, adoption, independent living and community outreach services to the children and families of Brevard County.
The Community Based Care initiative is currently being instituted throughout the State of Florida and nationwide. The CBC initiative in Brevard County is the collaborative effort of local child welfare agencies and community partnerships through the leadership of Together in Partnership, the Leadership Roundtable Alliance, Brevard County Government, Devereux,
and Children’s Home Society.
The partners voluntarily assumed responsibility for the health, welfare and safety of at-risk children and families in Brevard County. These agencies worked together for four years prior to implementation to increase prevention and early intervention services with the purpose of strengthening and preserving the integrity of the family while protecting the children. The CBC philosophy empowers and challenges neighborhoods and local communities to take ownership of their community by assisting children and families.
Since it began operations in 2005, Brevard Family Partnership has consistently ranked among the top performing CBCs in the state of Florida, ranking among the top three agencies over the course of the past five years in visits to children and families, timely reunification, and exceeding the federal measures for placement stability. Since the advent of Brevard Family Partnership, and the implementation of family-centered case practice, using the wraparound process, Brevard

| We are dedicated to bringing help in the toughest of times, offering encouragement and hope to those in the midst of a horrible battle. When a child is diagnosed with cancer it can devastate a family with fear and uncertainty. Brittany’s Wish is standing in the gap by helping ease the burden while spreading awareness. Assisting families financially and with other acts of kindness is our top priority. Families become overwhelmed and burdened by the extra financial strain and in most cases loss of income due to the need to be by their child’s side. We journey with our families throughout treatment. Our Cheer Gift Program provides a much-needed distractions from all the bad and scary stuff during treatment. Gifts are delivered to our young patients throughout treatment and siblings are included on their birthdays and Christmas. Siblings are also affected emotionally and are faced with many lifestyle changes. When a child completes their treatment, we help them celebrate with an End of Chemo celebration.

Our Mission
Crosswinds’ Mission statement, Crosswinds Youth Services creates opportunities for young people to succeed, embodies our organization’s primary goal of helping young people strengthen skills, develop leadership abilities, and reach their full potential.
Our History
Crosswinds began in the 1970’s with a group of concerned community leaders who came together to discuss how Brevard County, Florida could respond to the increasing number of runaway and homeless youth in the community. The tragic results of not helping these young people had become apparent when several graves were unearthed in the county. These graves contained the bodies of children who had run away from home and had been murdered.
The initial group of community leaders responding to this terrible tragedy included judges, a county commissioner, and Dr. Robert Lehton, a psychologist, who brought to the table his experience counseling runaway youth and establishing the first youth shelter in the state of Louisiana. Dr. Lehton is the Past Chair of Crosswinds’ Board of Directors, and the long-standing volunteer for whom Crosswinds’ shelter facility is now named. Out of the efforts of this group of volunteers, the organization now
known as Crosswinds opened an emergency youth shelter in 1974.
Over the years, Crosswinds has grown to include a wide range of programs and services for young people and their families. In early 2002, Crosswinds opened a new and expanded emergency shelter in Cocoa, Florida, moving the program from Merritt Island, where it had been located for nearly 30 years. Crosswinds administrative offices and other programs were also consolidated on the Cocoa campus to maximize resources and provide better accessibility to services for youth, families, law enforcement, and other community
organizations.
Crosswinds is committed to providing quality services and has been recognized nationally, regionally, statewide and locally. For example, Crosswinds was named 2003 Agency of the Year by the National Network for Youth, 2003 Organization of the Year by Brevard County’s major newspaper, FLORIDA TODAY, and recipient of the Melbourne – Palm Bay Area Chamber of Commerce’s Non-Profit of the Year Award in 2008.

An unfortunate product of the wars in Iraq and Afghanistan are the many thousands of men and women who, like generations before them, become wounded, injured or ill as a result of their service. For nine decades DAV has been empowering those veterans to lead healthy and successful lives after leaving military service.
Our mission simply is one of service and advocacy on behalf of the men and women who put their lives on the line to ensure our safety, to protect our freedoms and cherished way of life. What is right for our nation and our citizens is to take care of those who served. We do this by never forgetting their sacrifices. We do this by keeping our promise to them: to provide the resources and services they need to enjoy the best quality of life possible.
We are a strong and influential advocate for veterans and fight to ensure they receive the full range of services and protections they deserve. We accomplish this by providing free professional assistance to all veterans and their families in obtaining benefits from the Department of Veterans Affairs (VA) and advocating for their rights on Capitol Hill,and we do it all for free, without any government funding.
As an organization of veterans serving veterans, DAV has a proven track record of results. Each year we assist with more than 300,000 claims to obtain needed benefits through 100 offices throughout the United States and Puerto Rico. No other organization has helped more veterans lead high-quality lives after service than ours. Our National Service Officers counsel and assist veterans and their families in filing claims for VA disability compensation, rehabilitation and education programs, pensions, death benefits,
employment and training programs. They provide free services, such as information seminars and counseling and community outreach activities Our Transition Service Officers provide benefits counseling and assistance to service members filing initial claims for VA benefits. By filing compensation claims at separation centers where service medical records and examination facilities are readily available, we are able to provide prompt service to these future veterans.
Because the need is so great, DAV is assisting more veterans than ever. Our Mobile Service Offices are reaching veterans in small towns and communities far from our National Service Offices. We are offering our services and expertise to those men and women as they prepare to leave the military service and return to civilian life. Our Voluntary Services Programs are giving more aid to hospitalized veterans and community support than ever before. Our nationwide Transportation Network continues to log countless
miles helping injured and ill veterans get to their appointments at VA medical centers. Our Celebrity Entertainment Program is expanding to brighten the lives and bring comfort to those lonely veterans at VA hospitals.

ELC of Brevard provides oversight to early learning programs throughout the county. The Coalition holds agreements with childcare providers who offer School Readiness (subsidized childcare) and VPK services. The Child Care Resource & Referral (CCR&R) department provides information to the public about all early learning resources in Brevard. The Coalition's main objective is to assist families who are in need of quality childcare and early learning programs while ensuring that children have all they need
to succeed in school and beyond.

from the Executive Director
As the new executive director of Keep BrevardBeautiful, I know people want to live and do business in a beautiful community. Our residents and businesses are interested in cultural offerings, schools, entertainment, sports, and the environment, not only the trees-and-plants part of the environment, but the living environment. When people consider relocating here or opening a business and recruiting employees, the living environment of the community is a crucial component of their decision.
For the past 30 years, Keep Brevard Beautiful has worked tirelessly to keep our communities clean, with a focus on eradicating litter. KBB educates the citizens of our county about recycling and diminishing waste and strives to assist with attracting businesses and tourists to our lovely communities. Our goal is to cultivate an area our citizens are proud to call home and our visitors want to return to year after year.
As an affiliate of our national organization, Keep America Beautiful, we all work together to keep our communities clean, safe and vital today and tomorrow. In the months and years to come we will seek to further involve our youth in our programs. After all, they will be the ones to carry KBB's mission into the future.

| For 70 years, the Brevard Symphony Orchestra has presented inspiring performances of great music to audiences in Brevard and Indian River counties. The orchestra was founded in 1954 for a production of Gilbert and Sullivan’s operetta The Mikado presented by the Northrop Glee Club. Originally known as the Brevard Light Concert Orchestra, the group soon began presenting its own concert series under the leadership Frederick Jaehne. Brevard Symphony is the Orchestra-in-Residence for the King Center for the Performing Arts in Melbourne, Florida. Now in its 69th season, the BSO is led by Maestro Christopher Confessore. The North Brevard Charities Sharing Center is a non-profit 501 (c) 3, United Way supported Agency dedicated to helping the low income, homeless, aged, and all others in need in Brevard County.
The Growing Years:
We were organized in 1958 and chartered by the State of Florida in 1998. We are a non-denominational organization supported by donations from churches, private businesses, civic and fraternal clubs, United Way, and private citizens. This is supplemented with sales from our Thrift Shop.
We are governed by a Volunteer Board of Directors. Their goal is to ensure that this organization positively touches the lives of as many people in need as possible, those who might otherwise fall through the gap.
Where we're going:
Our Mission is to fill in the gap and provide emergency assistance to those individuals and families who may not otherwise qualify for assistance through other agencies. We help them to overcome barriers that may jeopardize their livelihood.

| Established in 1966, the North Brevard Historical Society is a non-profit.
The purpose of the society is "The collection, preservation, recording and display of historical artifacts, photographs and written records of an area to stretch from Port St. John to the Volusia County Line and from the Atlantic Ocean to the St. John's River."
The North Brevard Historical Museum was established in 1989. It is operated exclusively by volunteers and is funded through memberships, donations and fundraising events.
There is no charge for admission to the museum. Open daily, Wednesday thru Saturday; 12:00 - 4:00 p.m.

No two children are exactly the same. At TCS we are committed to providing your family with an atmosphere for academic achievement and advancement as well as a place where your child(ren) can grow in the Christian faith and develop their character.
What We Offer
TCS offers smaller class sizes allowing for more one on one instructional opportunities and help from the teacher. An average class is approximately 10-12 students to 1 teacher through third grade. An average class is approximately 18-20 students from fourth through twelfth grade. In grades K-6 we use Abeka curriculum exclusively. Secondary classes feature a variety of curriculum.

The Brevard Alzheimer's Foundation Story and Time line
Founded in 1995, The Brevard Alzheimer's Foundation is a not-for-profit 501(c)(3) organization. Donations and services stay in Brevard County, serve local residents and are fully tax-deductible. With an annual operating budget of $4.9 million dollars and over 80 employees, the Brevard Alzheimer's Foundation served over 2,000 families last year. Our organization is unique among other charitable organizations because over 90% of our revenue comes directly from services provided and not from fundraising.

| The Women’s Center is a 501c3 Non-Profit Organization dedicated to providing support, education, counseling and information. Hunger, homelessness, domestic violence and sexual assault occur daily in Brevard County. The Women’s Center is committed to addressing these needs by providing a progressive step towards safe, healthy, self-sufficient lives. Thanks to our volunteers and loyal supporters we are able to provide counseling services, financial assistance, career guidance, job training, transitional housing and victim advocacy. Our programs evolve to meet the changing needs in women’s lives. Every day, women come to us, often abused, battered and looking for advocacy, guidance and support. Helping women to help themselves means making it possible for the Women’s Center clients to not only support themselves, but to become active and proud participants in our community. BREVARD’S Domestic Violence Facts:
- In 2010, Brevard County experienced 6 Domestic Violence fatalities.
- 3,828 domestic violence offenses were reported to law enforcement in Brevard County. We know that many more victims are not reporting to the police or accessing services due to reasons such as shame, fear, or being prevented from doing so by their abusers. For this reason, we may never know the true extent of domestic violence in our county or in the state or country.
- In 2010, 1659 victims of Domestic Violence applied for an injunction for protection in Brevard County.
- The Women’s Center Victim Services provided assistance to 1224 victims in 2010 with safety planning, legal advocacy support, assistance with injunctions, with the address confidentiality program, and with free empowerment focused counseling.
Facts About Domestic Violence:
- 1 in 5 teens experience violence and abuse in a dating relationship.
- In the past 2 years, across the state of Florida, there is a 28.7 % increase in stalking. Stalking is often considered a precursor to sexual assault and domestic violence homicide. Perpetrators utilize a multitude of tactics to pursue and torture their victims including the utilization of technology to track the victims’ communication, whereabouts, and activities, as well as to engage in cyber-bullying and cyber stalking.
- The US Department of Justice estimates that 95% of reported assaults on spouses or e-spouses are committed by men against women. (Douglas 1991).
- Domestic Violence accounts for approximately 35% of all emergency room visits by women with injuries. (Randall T. Journal of the AMA ‘Domestic Violence Calls For More Than Treating Injuries’ 1992).
- Domestic Violence is the leading cause of injury to women between the ages of 15 and 44 in the United States. It’s more common than automobile accidents, muggings and rapes combined. (Uniform Crime Reports, Federal Bureau of Investigation, 1991).
- Women of all cultures, races, occupations, income levels, and ages are battered – by husbands, boyfriends, lovers and partners. (Surgeon General Antonia Novello, as quoted in Domestic Violence: Battered Women, publication of the Reference Department of the Cambridge Public Library, Cambridge, MA).
- Batterers can be successful professionals such as doctors, lawyers, corporate executives, salesmen, and teachers as well as skilled and unskilled laborers. (Based on Lenore Walkers theories explained in the book, The Battered Women, 1979).
- A batterer’s control of the family’s money can stop victims from ending the relationship or make the money situation harder for the woman if she leaves. (Peter Jaffe, David Wolfe & Susan Kaye Wilson. Children of Battered Women, 1990).
- There are three times as many animal shelters in the United States than there are shelters for battered women. (Senate Judiciary Committee Hearings. 1990).
- Injuries battered women receive are at least as serious as injuries suffered in 90% of violent felony crimes, yet under state laws, they are almost always classified as misdemeanors. (National Coalition Against Domestic Violence).

Our History
Park Avenue Christian Academy was founded in 1978 as a ministry of Park Avenue Baptist Church. Today we serve approximately 300 students in pre-school through 8th grade. We celebrate over 30 years of service to the families of northern Brevard County, and look forward to God’s plans for continued ministry. PACA has a heritage of academic and spiritual training that is respected in the community.

| The Merritt Island Wildlife Association (MIWA) is a 501(c) (3) non-profit friend’s organization partnered with the Merritt Island National Wildlife Refuge Complex. This complex includes the Merritt Island NWR, Saint John NWR, and Lake Woodruff NWR.
Our purpose is to promote conservation, awareness, and appreciation of these refuges and to provide assistance to refuge programs.
Since 1994, MIWA has relied on donations, membership dues, grant awards, and the generosity of private businesses and companies to help provide funds, materials, and resources to assist in our efforts. We also operate The Bookery, a nature-based bookstore located in the Merritt Island NW Visitor’s Center, with all proceeds going directly towards supporting refuge projects.
